I understand all appointments are conducted online—no in-person visits
I am seeking non-stimulant treatment for distractibility or executive function challenges
I understand Dr. Finney does not prescribe stimulants or controlled substances
I understand Dr. Finney provides treatment for symptoms, not diagnostic ADHD evaluations or accommodation letters
I am not enrolled in Medicaid (including Medi-Cal or MassHealth)
I can commit to the initial 6-month enrollment ($299/month, billed upfront; monthly after that)
I understand Dr. Finney does not provide general healthcare (antibiotics, sick visits, annual exams, etc.)
